Dedicated Focal Point – PCPC (People-Centered Primary Healthcare)-1 year contract duration

• Support roll-out of PCPC activities at PHCC level.
• Act as focal point for PCPC communication and coordination.
• Support dissemination and application of PCPC guidelines and tools.
• Assist in organization of PCPC trainings, meetings, and workshops.
• Follow up on PCPC implementation and identify challenges and good practices.
• Conduct field visits to PHCCs and provide feedback.
• Support documentation of lessons learned and implementation gaps.
• Submit regular progress reports and participate in coordination meetings.

5. Expected Outputs:

• Bi-weekly and monthly PCPC progress reports.
• Field visit reports and monitoring notes.
• Documented lessons learned, success stories, and challenges.
• Monthly report pertaining to the above activities and attendance schedule
