- Identify and assess vulnerable children and families through outreach, referrals, and hotline services.
- Conduct comprehensive psychosocial and protection risk assessments.
- Develop and implement individualized case management plans in collaboration with children and caregivers.
- Conduct regular home visits and follow-ups to monitor progress and ensure child safety.
- Coordinate referrals and follow-up with internal teams and external service providers (health, legal, MHPSS, education, etc.).
- Provide psychosocial support through individual sessions, group activities, and Psychological First Aid (PFA) when needed.
- Facilitate awareness sessions on child protection, safeguarding, health, and wellbeing topics for children, adolescents, and caregivers.
- Engage communities in mobilization, identifying at-risk individuals, and creating safe spaces for activities.
- Participate in team meetings, case conferences, and coordination with supervisors and partners.
- Maintain accurate, confidential documentation and case files; contribute to data collection and reporting.
- Adhere to safeguarding policies, code of conduct, and report any protection concerns or violations.
- Support family tracing and reunification efforts where applicable.
- Participate in organizational events, staff care initiatives, and internal development activities.
- Contribute to service mapping, safety audits, and feedback mechanisms to improve program delivery.
