Functions and responsibilities
In accordance with established policies and procedures, the Community Health Supervisor will:
- Manage and follow-up on the referral of patients to IMC Community Health Workers (CHWs), case management teams for mental health and psycho-social support (MHPSS) and GBV services, and to organizations working in other sectors such as education and shelter as needed.
- Work in close coordination with the community health supervisors (CHSs) and community health workers (CHWs) to strengthen the referral and follow up system of patients at the PHCCs and between the PHCCs and secondary healthcare facilities.
- Hold weekly health awareness sessions at PHCCs, or outside of PHCCs to reach patients who have difficult accessing the facility, targeting the Lebanese host community
- Work with clinic staff to ensure identification of vulnerable beneficiaries and those in need of additional assistance or case management
- Orient the patients to services available to them through IMC, UNHCR, and other stakeholders, and explain financial coverage and eligibility for PHCC services and refer as needed
- Follow up with pregnant women after delivery to encourage completing four antenatal care (ANC) visits and their return to the facility for post-natal care (PNC) visit
- Ensure that information on ANC & PNC is entered into the Health Information System (HIS) pregnancy record.
- Provide counseling and support for exclusive breastfeeding and immunization to newborns.
- Participate in public health assessments; oversee data collection; and compile data
- In absence of midwife, provide counseling on family planning and provision of modern contraception education
- Coach and support community health workers in utilizing data collection and monitoring tools
- Monitor outcome of referral with health facilities and provide feedback on referred cases to community health workers
- Monitor reporting for community-based health surveillance system
- Provide detailed reports to health area manager on a weekly basis
- Communicate information in a timely manner to report on community health outbreaks
- Assist in developing materials and visual aids for the program
- Attend team meetings and trainings as need be
