Non-Communicable Diseases (Ncd) Nurse - Tripoli

Summary and Background: The NCD nurse will be in charge of following up on all NCD services in the IMC-supported PHC facilities in his/her area. The NCD nurse will oversee all NCD services are implemented according to the agreed standards provided by the MOPH in addition to coordinating with the surrounding community.   Place in the Organization The incumbent is accountable to the Area Manager.   Functions and responsibilities In accordance with established policies and procedures and in coordination with the PHC facilities in the area, the NCD nurse will:   Clinical Supervise and follow up on all NCD services Support screening of all those above 40 years of age. Collect information for all current NCD patients to follow up with on regular basis Ensure the delivery of clinical care to patients with NCD based on MOPH standards, including high standard clinical practice and clinical expertise assessment, investigation, diagnosis and treatment. Ensure proper diagnostic tests for NCD patients are provided based on MOPH protocols and treatment updated regularly Ensure NCD patients receive chronic medications and allocate appropriate alternatives; report back on any issues in the system Strengthen surveillance system to monitor compliant patients, non-compliant patients and defaulters. Contact defaulters to come and follow up with their physicians. Review Medical Records to ensure proper implementation and documentation of NCD services Provide patient education for NCD patients and their families through individual and group sessions Report back to the respective clinic health officer on all issues related to the clinic regularly. Capacity Building Attend Capacity building activities to understand national guidelines for NCD care. Identify gaps in trainings and/or education of PHC staff including nurses, physicians, health educators and paramedical staff Coordinate with the team of NCD nurses and IMC central staff to prepare education material for staff Quality Comply with the professional practice standards for Nurses Regularly give feedback to the NCD policies and give input to quality initiatives regarding NCD services Regularly collect data and monitor quality indicators based on the guidelines Identify gaps in terms of implementation of NCD services and prepare work plans to implement quality improvement initiatives Prepare monthly statistics and trends to identify and evaluate effectiveness of the services on health outcomes. Fill all the reports and data required based on the project Conduct satisfaction surveys/ participatory assessments/ focus group to understand needs of the patients and input to NCD services. General Collaborate regularly with the respective health officers and medical advisors to ensure implementation plans are in line Regular meeting with NCD nurses to discuss challenges, recommendations and updates Perform any other tasks as requested by line supervisor
