Outreach Social Worker/case Manager

Provide social      support to displaced Syrians and their families at the primary health      clinics , home and shelters level; Conduct      community outreach and home visits as well as visits to other centers and      PHC with the area of work; Work directly  with the case management team including psychologist, psychiatrists and field officer to develop intervention plans and follow up at the home/ shelter level; The case manager will be an essential source of providing accurate referrals when need be; i.e., the case manager will refer patient to hospitals and centers; in addition to other local NGOs and INGOs.  Complete  intake assessments, evaluations, reviews, and other documentation as  necessary. awareness  raising activities and sessions at the primary health clinics , home and  shelters level Attend weekly or biweekly meetings with North based MH officer. Identifs   families or individuals in need of psychosocial support or counseling  through home/shelter visits. Collect information about visited vulnerable families through interviews,  observation, and tests carried out via outreach home visits. Attend the  technical meetings for the area team and other required meetings with the  area based MH officer and the community center manager; Reporting: Reports directly to IMC area MH officer. S/he will submit the following reports at the end of each week: The Case Manager Report; The Narrative Report which includes: the number of beneficiaries, actions taken, number of house-visits conducted, awareness raising activities, number of people attending the session with breakdown per age group and gender; Weekly update   – numbers of beneficiaries/patients segregated by age, gender and type of service provided (i.e. mental disorder, GBV survivor, other); Qualifications: BA in Medical and/or Social work; Minimum 3-5 years in the field of community outreach and social work; Previous work experience in mental health detection and referrals; Minimum of 2 years of previous NGO experience; Fluent in Arabic – knowledge of English is a plus; Ability to manage cases   in privacy and total respect of human right and work ethics; Living in area  of work.
