Outreach Social Worker/case Manager

      Program Title: Comprehensive Iraqi Refugee Assistance Program Position: Outreach Social Worker/Case Manager Location:  IMC  Haret Hrek BSS   Timeframe: 10 months Tasks & Responsibilities: Social Worker Duties The case manager will be an essential source of providing accurate referrals when need be; i.e., the case manager will refer patients and clients to IMC supported clinics, hospitals, and centers; in addition to other local NGOs and INGOs.  Moreover, the case manager will dispense information about the various programs and services offered by IMC and its affiliates.  The case manager will be in-charge of dispensing information about returns to Iraqi refugees and families wanting to know more about the correct procedures and required documents needed to commence the process of returning back to their native land.  Undertaking and writing up assessments, which meet specified standards and timescales;      conducting interviews with service users (Iraqi Refugees and host population) and their families to assess and review their situation; Liaising with, and making referrals of specific cases to IMC Maintaining accurate records and preparing reports for proper information flow and  legal action; Participating in training, supervision and team meetings. Identify families or individuals in need of psychosocial support or counseling through home visits to Iraqi refugee      households. Collect information about visited vulnerable families through interviews, observation, and tests carried out via outreach home visits. Encourage families to express their feelings and discuss what is happening in their lives, and help them to develop insight      into themselves and their relationships. Early Childhood Development (ECD) and Youth Empowerment Program (YEP) Recruit Iraqi and Lebanese caregivers through clinic based work and outreach support to families, for participating in Early Childhood Development Program. Be the lead facilitator of the ECD sessions based in the Beirut Southern Suburbs. Recruit Iraqi and Lebanese youth through clinic based work and outreach support to families, for participating in Youth      Empowerment Program. Be the co-facilitator of the YEP sessions based in the Beirut Southern Suburbs. Complete intake assessments, evaluations,treatment recommendations, reviews, and other documentation as necessary. Reporting: Write monthly reports to be submitted regularly to IMC program Director and Program support officer, providing timely updates as required Fill in monthly case management tracking form Goal setting forms as advised by the Mental Health Coordinator
