Outreach Social Worker/case Manager For Halba, Aakar

JOB DESCRIPTION Outreach and in clinic Social Worker   Program Title: Syrian Refugee Assistance Program   Position: Outreach Social Worker/Case Manager   Location: Halba Akkar                                                         Timeframe: 12 months   Tasks & Responsibilities: Provide social support to wounded displaced Syrians and their families at the hospital level. Work directly with the psychologist to develop intervention plans and follow up at the home level. Complete intake assessments, evaluations, reviews, and other documentation as necessary. The case manager will be an essential source of providing accurate referrals when need be; i.e., the case manager will refer patient to hospitals and centers; in addition to other local NGOs and INGOs.  Liaising with, and making referrals of specific cases Maintaining accurate records and preparing reports Attend weekly or biweekly meetings with North based MH officer. Report directly to IMC regional mental health coordinator. Identify families or individuals in need of psychosocial support or counseling through home visits. Collect information about visited vulnerable families through interviews, observation, and tests carried out via outreach home visits. Encourage families to express their feelings and discuss what is happening in their lives, and help them to develop insight into themselves and their relationships.   Reporting: Submitting the following reports to the North IMC Psychosocial Field Officer at the end of each month: The Case Manager Report The Narrative Report [includes number of beneficiaries and house-visits conducted] Other reports requested by the North IMC Psychosocial Field Officer   Qualifications: Social Work background Minimum 2 – 3 years in the field of social work Previous NGO experience is a plus Fluent in Arabic – English is a plus Adheres to ethical considerations and principles in regards to working with vulnerable populations. Preferable to be from Halba – Aakar.
