School Case Manager - Bml

The SCM is responsible to the community health activity in schools and will be in regular contact with School Community Health Workers, and the school health educators to oversee the community health services provided for students and their families in outreach level. The incumbent of this position will report and work under the direct supervision of the community health officer (CHO) in the implementation of the school community health project.   Place in the Organization   The Community Health Educator will report directly to HEO.  Furthermore, the CHE will oversee the work of Community Health Workers in the area. Primary Responsibilities   With guidance from the CHO and in close collaboration with grassroots field teams, the incumbent conduct assessments, awareness sessions, referrals as well as coaching of the community health workers in the International Medical Corps’ School Community Health Project in Beirut and Mount Lebanon. Responsibilities of the School Case Manager: Assess the community health needs for the students and parents. Communicate and report community health needs, concerns and resources with the CHO and the CHPM. Assist in the development of community health education and promotion activities such as training workshops, conferences, and school or community presentations. Set a schedule of trainings and activities and prepare for the implementation of the community health outreach network with the school based health educators to emphasis the integration of the parents in the health education program under the supervision of the CHO. Suggest new capacity building training topics for the school based health educators based on the needs. Collaborate with the CHO to set the qualifications and requirements of the persons who will be selected among the parents for outreach community health education activities. Prepare with the CHO reporting tools for the school health educators and outreach workers to submit on a weekly and monthly basis. Provide on the job training and supervise outreach workers on correct community health interventions. Prepare with the CHO tools to evaluate the effectiveness of the sessions provided at schools, such as knowledge assessments and pre/post tests. Get engaged in distribution of health information materials, such as brochures, posters, kits… Develop, and coordinate health needs assessments and referral pathways to primary health centers. Perform other duties and tasks as requested by line manager/the management. Adhere to all of International Medical Corps’ policies and procedures, including Code of Conduct and Security SOPs.
