1. Case Management Process
- Conduct initial assessments to determine client needs and priorities.
- Develop and implement tailored case plans in collaboration with the client.
- Monitor and evaluate progress, adjusting plans as needed.
- Facilitate referrals to appropriate services, including healthcare, shelter, education, and psychosocial support.

2. Beneficiary Support
- Provide ongoing emotional and practical support to clients.
- Advocate on behalf of clients to ensure access to services and rights.
- Empower clients to build self-sufficiency and resilience.
3. Coordination and Collaboration
- Work closely with internal departments, external partners, and service providers
to deliver comprehensive support.
- Participate in case conferences and coordination meetings to ensure integrated
service delivery.
4. Documentation and Reporting
- Maintain accurate and up-to-date case files and documentation.
- Prepare timely reports on case management activities, including progress and
challenges.
- Ensure data confidentiality and secure storage of client information.
5. Capacity Building and Awareness
- Provide training and awareness sessions for staff and beneficiaries on relevant
topics.
- Stay updated on case management best practices and sector-specific guidelines.
