Social Worker (Two Positions: Jal El Dib And Akkar)

Main Role The Social Worker will be working the LRC MS Centers. He/ she works with people to support them through difficult times and to ensure that vulnerable people, children and adults have access to health care. In addition, the social worker provides support in enabling patients to help themselves improve their lives by working with their surroundings. Moreover, the social worker contributes to the implementation and monitoring of specific projects / programs within the center as needed.    Essential Responsibilities and Duties   Performs initial screening and evaluation of patient and families for vulnerability in relation to set criteria to assess and review their situation Collaborates with members of the health team, the patient/family to assure patients’ psychosocial and socioeconomics needs are addressed wherever possible Counsel patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life Monitor, evaluate, and record patient progress according to measurable goals described in treatment and care plan Collaborates and refers patients as necessary to Branch Doctor or any external medical center as needed Educates patients and families on the roles of social workers; assists patients and families in communicating with one another and to social workers Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the patient Interprets- translates information related to medical treatment plan to patients as needed Coordinates patient discharge and continuity of care planning Recognizes patients with underlying mental illness, provides and/ or makes necessary referrals
