Main activities

• Conduct the medical assessment for patients and administer urine tests

• Assess the case medically before each psychiatrist visit
• Fill the MH nurse intake and progress notes forms
• Psychoeducation for patient and care givers about the medication, HIV, and Hepatitis B and C
• Administer urine tests for all SU patients
• Administer injection when required

• Follow up medication reconciliation, lab tests and medication stock

• Remind the case managers about the next psychiatric appointments of the patient
• Follow up the tests recommended from the psychiatrist with CM and patient
• Ensure that weekly OST prescriptions are correctly filled and comply with MOH protocol
• Coordinate with the Pharmacist about the medication stock and consistently inform the CMHC team for any shortage in medications

• Responsible for the patient file

• Ensure confidentiality of the patient’s file
• Responsible for safekeeping doctor prescription pads and stamps

• Responsible for the link between psychiatrist and external CM/ Therapist

• The nurse will be responsible of all the stable cases followed by the psychiatrist and referred from CM
• The nurse will manage the link between patients that are followed with other NGO/INGO and are referred only for psychiatric purpose
• The nurse will coordinate with the receptionist and CM for the psychiatrist appointments
• The nurse will work in coordination with the team and will be part of the technical meetings / case discussions
