Home Base Care supervisor

As Home Base Care supervisor, planning, implementing, coordinating and supervising the home base care activities of the project according to MSF values, standards, procedures in order to provide the most appropriate model of care for patients with non-communicable diseases in Ain Al Hilwe camp. (~50%).

As outreach nurse or social worker, perform patients follow up in the field according to the team plan. (~50%).

 

  • Assist and support the project coordinator in the implementation and follow up of the overall medical content, activities and resources in the project. Facilitate the proper medical/social follow-up of the patients enrolled in MSF Home base care program.

 

  • Participating actively in reflections and discussions on the evolution of MSF protocol on Home Base Care program in the project, contributing with his/her background and experience to the improvement of overall performance, adapting it to the changing reality and context together with the project coordinator.

 

  • In support of the project coordinator, representing MSF in meetings or with different stakeholders. Assisting the project coordinator in the stakeholder mapping and context analysis.

 

  • Participating in the collection of medical and social information (meetings, reports, articles, etc.).

 

  • Ensure weekly data collection on medical activities and monthly-quarterly reporting of activities according to the expected standard and deadlines. Ensure that each medical team member handles patient data, patient files confidentially, and that patient information and files are kept in a safe, locked place.

 

  •  Maintain a referral pathway for the patients in need of services not provided by MSF in order to propose the best support available. Advise the medical team and the project (project coordinator) on availability of services.

 

  • Provide input and advise the medical team on the building up of the social profile of the patients assisted by MSF (including during home visits) allowing good understanding of the social and medical condition of the patient. 

 

  • Participate in the creation of any materials needed for patients’ education and monitoring the tools to improve the Home Base Care services.

 

  • Regular visits to the communities and patients that are under his/her responsibility in order to assess and report actively on the most urgent needs at any given time.

 

  • Participate to the case study with the nurse, the Medical Doctor and social worker. Suggest solution to address the social issues the patients if facing.

 

  • Follow up the NCD medicine stock for patients at the project level

 

  • Participate to the identification of any gaps in the Home Base Care model and assist the Project coordinator in advocating for this model with relevant parteners.

 

  • Report any social/medical/context evolution and report this information directly to the Project Coordinator

 

  • In collaboration with the Project coordinator follow up and supervise the organisation/time allocation of the Eprep activities plan conducetd by one med team member
