- Conducting home visits with the doctors to perform medical consultations to patients enrolled in the Homecare service – where physical assessment, blood pressure, blood sugar monitoring, dressing/wound care, etc…
- Responsible for the medications dispensed to the patients and documenting on stockcards to keep the inventory of medications updated
- Reporting on cases seen, cases in need for hospitalisation, etc…
- Data entry on the patients’ files (hard copies)
- Referring patients in need for services (mental health, nursing care, kits, protection, elderly home, food, etc…) to the social workers and following up on them with the social worker
- Referring patients in need for hospitalisation and/or Lab requests to the MTL
- Responsible for following up on patients’ lab results along with the doctors
- Patient education on diseases and medications
- Data entry of patients’ files when/if needed
