1. Summary of the post Perform consultations and medical follow up for patient enrolled in the Chronic Disease program, according to the MSF treatment. Ensure the strict adherence to the lab investigation according to the MSF calendar In charge of the clinical follow up of the referred cases to secondary health care, according to the referral criteria of MSF. This activity include all the referrals sent to an identified hospital by MSF, according to a specified agreement. The MD responsible for referral must abide by the points enlisted in the agreement. Manage the daily dialogue with the hospital medical doctors React and inform the Medical Focal Point for any important issue arising from the referral to Provide timely and complete report about the medical Chronic Disease management, as well as the referral outcome and figures     2. Main activities   a)       Daily Consultations in the afternoon time for patients with chronic diseases   b)       Follow up of the Referral of Severe cases in the morning time. In this activity is also included the documentation of the consumption of drugs and report for supply to the nurse.   c)       Communication Ensure good communications with line manager for reporting for severe complications of chronic disease patients, Ensure Patient friendly behaviour attitude  with patients and care takers/ family members Ensure teamwork spirit within mobile health clinic staff, by assisting and giving good guidance to his/her colleagues    d)       Hygiene and infection preventions control Follow the universal precaution (asepsis and hygiene) and the waste management procedure (sharp waste, contaminated waste).   e)       Training/ Meetings   Attend training and meetings done by MSF.
