Patients Affairs
- Ensure follow-up of patients in hospitals, regarding treatment & outcome by conducting frequent visits
- Segregate data from the assessments and refer them: Medical Team, Nurses Team, external stakeholders, and/or Laboratory
- Organize the waiting list of patients to be seen by team/area
- Prioritize cases that need urgent referrals: internally or externally
- Update patients’ list on a daily basis marking those Seen vs. Not Seen and share it with the relevant teams
- Remind the medical team of the patients who have follow-up visits coming up
Referrals
Healthcare Facility
- Ensure laboratory (requests vs. tests done) match
- Ensure that the pledge form exists and attached with all hospital bills and the ceiling amount presented is abided by unless there was an approval to exceed it
- To maintain a network of hospitals for referrals according to MSF’s criteria of admission (life-saving)
- Identify non-conformities in the bills, and report them
- Maintain good communication and relations with the referral hospitals & their representatives, and report any concerns or problems directly
- Collect bills and invoices from the laboratory and hospitals by the end of every month
Service Providers
- Ensure that patients in need for services outside MSF are referred to the relevant service providers and are followed-up on until the case is closed
- Incoming referrals from other service providers are forwarded to the Roving Volunteers, and then the process takes the normal procedure from there on
Reporting
- Data entry on the Referral Database with accurate data issued from service providers, or from the staff in the field and update it on a regular basis following feedback on the service provision
- Follow-up with the patients and get their feedback for documentation
- In charge of the weekly referral report
- Maintain quality of data collection for patients referred to secondary health care, including indications, treatments, outcomes, costs etc.
- Provide relevant analysis of the referral collected data and produce information on project’s outputs, outcomes and impacts
- Update the waiting list accordingly
- Ensure timely and correct data entry of the: patients who fit criteria segregated from the Data Officer, referred and seen by the Medical Team, referred to the health facilities, to the Nursing Team, and follow-up on patients not seen, etc.
Follow-up on the requests sent to the Nursing Team and give feedback to the medical team
