Key responsibilities  

Accountability and Complexity 

Under the supervision and guidance of the Public Health Officer, RI nurses will monitor the referral process for elderly patients with non-communicable diseases (NCDs), ensuring they receive appropriate care and treatment. 

Complex Care Coordination: RI nurses will coordinate between the supported PHCC and the hospital, managing multiple aspects of care for elderly patients, particularly for NCD-related conditions, from initial consultation to hospitalization and prost discharge follow-up. 

Effective Communication: RI nurses must possess excellent communication skills to liaise effectively between elderly patients, healthcare providers at PHCCs, and hospitals, ensuring the proper flow of information and continuity of care. 

 

Patient Care at Al Iman PHCC and Hospitals 

 

Receive Referrals for NCDs: Assess referrals of elderly patients with NCDs, and coordinate with the Field Medical Coordinator for approval and further management in secondary health care settings. 

Coordinate Referrals for NCD Management: Facilitate referrals to the supported Hospital for further evaluation and management of NCDs. Ensure that referrals are timely and that patients are fully aware of their treatment plans. 

Monitor and Follow-Up: Continuously track the progress of elderly patients with NCDs through regular follow-ups. Ensure that patients adhere to prescribed treatments, attend specialist appointments, and receive the necessary care to manage their conditions effectively. 

Facilitate the Admission and Treatment of NCD Patients: Coordinate with the supported Hospital to ensure elderly patients requiring hospital admission for NCD-related conditions are promptly admitted and receive appropriate interventions. 

Ensure Comprehensive Documentation: Ensure full documentation by hospital staff for all patient interactions related to NCD management, including medical histories, care provided, and follow-up visits. Maintain accurate copies of medical files and referral documents to support continuity of care and effective coordination with health facilities. 

 

Coordination  

 

Ensure Continuity of Care: Communicate effectively with the supported PHCC to ensure elderly patients are receiving the necessary follow-up care for their NCDs. 

Provide Education and Resources: Identify gaps in knowledge or practices related to NCD care at the PHCC and coordinate with the Public Health Officer to develop and implement training plans to enhance capacity-building efforts for PHCC staff in managing NCDs. 

Work jointly with the medical administrative to monitor proper implementation of a treatment plan at the hospital, along with medical bill monitoring.
