Health Community Mobilizer

KEY AREAS   OF ACCOUNTABILITY:      Carry        out activities according to the implementation plan    Work        directly in the field with children, families and other stakeholders of        the project location he/ she is allocated to    Identify        new beneficiaries (registered and non registered) in the location he/she        is allocated to on an ongoing basis    Advise        beneficiaries on existing health services and other SC sector activities        and refer them to relevant organizations if needed    Follow        up and review beneficiaries who attended the health facilities    Collect        information on the health needs through home visits, presence at health        centres, and communication with stakeholders    Provide        information to beneficiaries on key health issues such as hygiene,        pregnancy care, new born child care, and identification of illness.     Organize        and carry out awareness sessions for children, adolescents, parents and        communities.    Support        health and nutrition team in trainings as required.    Participate        in health and nutrition assessments/mapping as required   In case of acute health emergencies such as   disease outbreaks, be part as response team involved in e.g. appropriate   vaccination programmes, case management and control of spread.     Liaise        on a daily basis with partners and communities to guarantee the smooth        operation of the programme.    Collect        data, document and report according to monitoring plan.
