Objective 1: Responsible of the Case Management Process within his or her perimeter, through the management of a team of XX Case Workers
- Reviews staff caseloads to ensure they are manageable and share challenges with senior management;
- Supports individual cases where required and provides regular monitoring of all aspects of case management services;
- Ensures that staff gaps do not result in breaches in the support to beneficiaries, and support CM Expertt in addressing such issues in the capacity building plan;
- Facilitates and attends case conferences for complex cases. Ensures case conferences are called involving relevant stakeholders;
- Ensures access to activity material and logistic support, and set eligibility criteria for material in collaboration with the Project Manager;
- Provide briefings on child safeguarding and appropriate behavior with children to all case management staff. Ensure any concerns are reported and addressed immediately;
- Monitors the case management process using the supervisor’s supervision and monitoring tools;
- Ensures the implementation of the internal and National SOPs, guidelines, referral and coordination mechanisms for assistance to children;
- Ensures that the identified and referred cases are followed up in an agreed time frame.
- Assess fairly and adequately for the cash provision assistance to the most vulnerable and marginalized groups.
Objective 2: Ensure the dissemination of information from senior management to CM team;
- Conducts weekly case management meeting with CM team to ensure the dissemination of important information from the field and other sectors in the area;
Objective 3: Assists the CM Expert in developing capacity building plans;
Objective 4: Enhance the capacity of TDH IT to safely identify and refer CP and other vulnerable cases.
Objective 5: Support the CM Expert in providing technical advice to case workers and senior caseworker on the identified cases;
Objective 6: Reports, Capitalization and Communication
- Ensures and monitors the correct documentation of cases through review of files and databases. Ensure confidentiality of information, and that data protection and information-sharing protocols are respected;
- Reviews, analyzes and reports on trends in the case management to inform programming;
- Assist the CM expert in finalizing quarterly reports for Case management implementations
- Trace, Monitor and report the Hotline
Objective 8: Support, Coach, and Monitor TDH IT’s Partners
- Review the received databases
- Supports individual cases where required and provides regular monitoring of all aspects of case management services;
- Ensures that staff gaps do not result in breaches in the support to beneficiaries
- Monitors the case management process using the supervisor’s supervision and monitoring tools;
- Train the partners on the different relevant and needed topics
- Coach the partners in implementing the relevant case management steps that as in-line with the National SOPs
Objective 7: Manages the team
- Supports the recruitment of staff for field positions when needed;
- Gives realistic yearly / Program objectives;
- Follows up on the tasks assigned to the team / development plan;
- Organizes Formal and Informal, Individual and Group meetings;
