Responsibility (process and functions undertaken to achieve results) Referral care includes: To review cases received from partners -To review proper documentation and complete information of cases as per SOPs -Based on medical professional capacity, directly respond to the partners and liaise with them Accordingly to respond back to partners within 48 hours, informing them of UNHCR’s decision -Follow up on cases when partners do not reply accordingly -Ensure all medical procedures are done using MoPH flat rates. -Keep a record of the cases he/she reviewed using a similar data base to that of the Senior PH Assistant's and update it regularly and share with the Senior PH Assistant's own existing database. Adhere to patient confidentiality Assist with compiling and filing of incoming cases Refer back to the Senior PH Assistant and PH Officer for additional guidance and for any exceptional cases which may arise. Compile cases for the Exceptional Care Committee meetings held twice a month. Provide valuable input for the revision and redrafting of the SOPs and other valuable documents.Authority (decisions made in executing responsibilities and to achieve results) Compile all necessary data for submission to ECC for urgent cases. Follow up with partners for collection of full medical reports on a timely manner. Communicate decision of ECC if tasked to.
