How to Apply
The Supplier shall complete and submit the following documents with his quotation:
- Request for Quotation Form - to be signed and stamped by the Bidder
- Annex #1 Technical Data Form - to be signed and stamped by the Bidder
- Annex #2 Financial offer (Price to be inserted by the bidder) - to be signed and stamped by the Bidder
- Annex #3 SUP 12 General terms and conditions for supply contracts.
- Annex #4 Code of Conduct for Contractors
- Annex #5 Vendor Bank Verification Form signed and stamped by the vendor
- Letter from the bank, Stamped and signed by the bank, for confirmation of the bank account details. It should be issued in 2025.
- Copy of a valid commercial registration issued in 2025.
- Copy of the ID of the company owner.
- Physical Samples for all the requested items, to be exact to the requested specifications.
Interested Bidders can also request Annex #5 Vendor Bank Verification Form and the financial offer template in excel by email.
