How to Apply

The Supplier shall complete and submit the following documents with his quotation:

  • Request for Quotation Form - to be signed and stamped by the Bidder
  • Annex #1 Technical Data Form - to be signed and stamped by the Bidder
  • Annex #2 Financial offer (Price to be inserted by the bidder) - to be signed and stamped by the Bidder
  • Annex #3 SUP 12 General terms and conditions for supply contracts.
  • Annex #4 Code of Conduct for Contractors
  • Annex #5 Vendor Bank Verification Form signed and stamped by the vendor
  • Letter from the bank, Stamped and signed by the bank, for confirmation of the bank account details. It should be issued in 2025.
  • Copy of a valid commercial registration issued in 2025.
  • Copy of the ID of the company owner.
  • Physical Samples for all the requested items, to be exact to the requested specifications. 

Interested Bidders can also request Annex #5 Vendor Bank Verification Form and the financial offer template in excel by email.
